CLW Support, Information & Guidance
‘Scotland’s most deprived communities need additional support, so we will recruit at least 250 Community Link Workers to work in GP surgeries and direct people to local services and support’ – A Plan for Scotland, SNP Manifesto 2016
ScotPHN is supporting Health & Social Care Partnerships and their partners to develop and implement Community Link Workers (CLW) in order to tackle health inequalities, improve health and well-being and reduce pressure on general practice.
On this page you will find links to information and guidance about CLW; recruitment and selection: induction and core training; data and information sharing agreements; role of HSCP’s, standards and governance: and monitoring and evaluation.
